Enrolled Students
Now that you have been accepted and paid your enrollment deposit, you will be receiving the following forms to become a Quincy University student:
Roommate Assignment Questionnaire
Used to determine compatibility of students for residence hall room assignments.
Emergency Medical Form
Form used by the Office of Student Affairs in case of a medical emergency. Requests emergency contact information.
Publicity Form
Used by the Office of Marketing and Public Relations to send information about student achievements to hometown and Quincy area media outlets. Students may choose "No Publicity" if they do not wish for such information to be made public.
Registration and Advisement Registration (RAP) Form
Form to reserve your place for RAP. RAP is a daylong event for new students to register for classes with an academic advisor, take various placement tests and confirm financial aid packaging. (Forms are sent in March.)
