Transcript Request
Due to graduation, transcripts ordered from the Registrar’s office will not be processed from May 9-May 23, 2008. We will resume sending out transcripts after May 23.
You may request transcripts within this time period, and we will resume processing the requests on May 23, 2008.
Transcript Requests may be made in the following ways:

Mail a completed request to:

or fax a completed request to the Registrar's Office at
(217) 228-5283
or come in to complete a Request for Transcripts form, available in the Registrar's Office, FRH 130.
NOTE: In order to comply with FERPA, e-mail or phone requests can not be processed. When requesting a transcript, please provide the following:
- Name
- Maiden Name (if applicable)
- Social Security Number
- Date of Birth
- Dates of Attendance
- Current Address
- The address to where you would like the transcript sent
- Your Signature (requests will not be processed without your signature)
Please allow 3-5 working days for processing. Please allow at least 2 weeks when requesting at the end of a semester.
Effective 12/1/2007 – The transcript fee will be $5.00 for each transcript requested. Payment must be made before transcript(s) will be released.
PLEASE NOTE: A transcript request cannot be proccessed if the Business Office has a hold on your account.
An official transcript has both the university seal and the Registrar's signature. The Registrar's Office can mail an official transcript to you, but it must remain in the sealed envelope to be considered official. Some institutions will only accept a transcript sent directly from the Registrar's Office. |
